Why do people say "the Ukraine" instead of "Ukraine"? How did this error begin and/or become so widespread?

At one point is was a region of Russia and was referred to the Ukraine but now days it is actually correct to call it Ukraine, but most everyone I know still calls it the Ukraine and these people are Ukrainian.
